Rethink Mental Illness is part of the Somerset County Council's Pathway for Adults service.

Somerset's Pathway for Adults provides an integrated range of services to meet the needs of vulnerable people who are single, have a history of homelessness, poor mental health and or offending background and are in need of accommodation or "end to end" support to prevent homelessness and develop long term independence.

Rethink Mental Illness provides accommodation and support for single vulnerable people who are experiencing poor mental health and are at risk of homelessness and are residents or have famly links in Somerset.

Services we Provide

Overview

Rethink Mental Illness provide accommodation, outreach or resettlement services for single vulnerable people who are experiencing poor mental health.

Vulnerable people are provided with a targeted support service which is individually tailored and which will focus upon attainment and sustainment of appropriate accommodation and integration into the community.

What we do

Rethink Mental Illness (as part of Somerset's pathway for adults with mental health service) aims to:-

  • Sustain independent living for adults with mental health conditions.
  • Reduce those with a mental health condition being at risk of homelessness.
  • Manage crisis.
  • Provide an outreach service to identify and support clients to attain long term accommodation.
  • Support clients to reduce their presenting needs and to access and sustain engagement with services to support their ongoing needs.

Rethink Mental Illness provides a person centric approach to support  by placing the individuals views at the centre of all support planning and develop the client's capacity to live independently.

Where we do it

Rethink Mental Illness has two accommodation hubs and two high need mental health accommodation units together with resettlement and outreach teams.

Location of these hubs are in:

  •  P4A East at Keward House in Wells and is the single point of access for all services across Mendip and South Somerset.
  • P4A West at Eastleigh Court in Taunton and is the single point of access for all services across Taunton Deane, West Somerset and Sedgemoor.

Rethink Mental Illness provides accommodation and outreach and resettlement support across Somerset for individuals with mental health issues.

How to access

The selection criteria for prospective clients to Rethink Mental Illness P4A is as follows:

  • Applicant will have a significant history of mental ill health and will be considered in relation to their needs: level of mental illness; need for support to work towards recovery; plan for independent living; homelessness; unsatisfactory housing or need to move from residential care, rehabilitation care, acute in-patient care or need for supported accommodation.
  • Potential clients will not be in an acute state of mental ill health at the time the application is considered.
  • Rethink will consider people with dual diagnosis of mental illness and substance misuse under the conditions specified in the substance misuse policy.
  • Clients will be over 18 (16 or 17 years will be considered in exceptional circumstances).
  • Clients will have a commitment to accept support as part of a recovery based support plan process.
  • All prospective clients will be assessed in relation to risk factors appropriate to the staff support that is available through the service.

Referrals can be received from a variety of sources including self, carers, professionals and partners.

Referrals can be taken over the phone between 9am and 5pm Monday to Friday by telephoning:

East P4A on 01749 676148 or West P4A on 01823 276075.

A referral pack will be sent out.

We can also provide advice and signposting to services for individuals with mental health issues.

What we do

Rethink Mental Illness provides integrated mental health services that include outreach, accommodation and resettlement support and works in partnership with Langley House Trust, Taunton Association for the Homeless and Homegroup.

The P4A Hubs serves as the main access point for referrals for support or accessing an accommodation element and will provide advice and information on a range of issues including signposting to other services.
