About P2I Pathways to Independence
Pathways to Independence (known as P2I) are providers and partners working together to make a real difference to young people, aged 16 to 24, across Somerset. P21 aims to provide information and advice around housing for young people, mediation and reconcillation services, short term emergency accommodation, supported and move on accommodation.

Overview
Pathways to Independence (known as P2I) offers housing and advice for young people aged 16-24 across Somerset. It is a jointly commissioned initiative by Somerset County Council, District Councils working with a consortium of provider organisations.
What we do
P2I provides a range of solutions and other servcies aimed at meeting current needs and positively reducing future needs. This includes:
- Homelessness Awareness and Targeted Prevention work with local schools and colleges.
- Specialist information and advice at the Hubs, based on a one-stop shop model with access to District Council Housing Option Teams, County Council social workers and experienced and knowledgeable provider staff.
- Floating support aimed at enabling young people to sustain their current accommodation and avoid homelessness.
- Mediatation and reconcillation services aimed at enabling young people to stay in the famliy home and/or improve family relationships.
- Short stay emergency accommodation in supported housing projects or with host families for those young people with an urgent accommodation need.
- Supported accommodation for those who need both accommodaiton and support.
- Move on accommodation for those ready to move on from supported housing or those who, from the outset, require accommodation with a lower level of support.
- Resettlement support for those who move on to independent living but need some support to help through that transition.
Where we do it
There is a P2I Hub in each District area serving as a main access point for young people and will provide advice and information on range of issues and provide assessmnet, co-ordination and support to all those young people who need it.
After a triage process, each young person entering the Pathway will have an integrated and holisitc independence plan agreed with them that reflects their individual needs, their preferences, their strengths and their ambitions. For some young people, their plan may involve the need to access accommodation based services, for many it will not.
